By, signing this roster/Registration/Waiver Form, I am a player acknowledge that participant in this activity involves a risk of personal injury my property, and I knowingly and voluntary agree to the terms and conditions outlined in this Waiver and Release from Liability. In exchange for being permitted to participate in this activity, I agree to the Following: I acknowledge that soccer is a contact sport and that there is a risk of injury from participating in the tournament. I HEREBY WAIVE AND RELEASE, North Newark Soccer League, Newark City, Irvington City and Essex County and its agents, servants and employees from any liability claims for damages. In the event of an emergency, I hereby give permission to such Medical personnel as necessary to render treatment. By signing this document, I also elect to adhere to the rules and regulations of the tournament.
